At registration you will need to bring your child's Birth Certification verification. We will also require the following items to complete your registration:

• 2017-2018 End of Year Report card - this cannot be provided until the school year is complete.
• Pop Warner Physical form completed on or after January 1st, 2018 - this cannot be completed by a nurse or chiropractor
